Requesting Your Score Report

If you would like to request another copy of your score report after you have already received your original report, please fill out the form below.

When filling out the form, please take note of the following:

• The process of receiving a new copy of your score report is NOT automated and will typically take 1-3 days to process. Please factor in this timeline when requesting scores. If you need your scores urgently (i.e. within the day), please reach out to us directly.
• The score report is NOT an official BYU Transcript. The credits you earn will be added to your transcript at the school you are/were attending. If you have any issues transferring your FLATS credits, reach out to us and we will get in contact with your institution.
• If it has been less than 7 days since you took your FLATS test, please wait until it has been 7 business days before requesting a score report through this form.
